Release and Waiver of Liability

I, parent/guardian of the youth named above, hereby give permission for my child to participate in the Open Bible Baptist Church youth group trip to Summit Student Camp at Ponderosa Retreat & Conference Center. I understand that the event is an off-site trip leaving from Open Bible Baptist Church (OBBC) on Monday July 6, 2026 and returning to OBBC on Friday July 10, 2026. By choosing Yes, I hereby release, waive, discharge and relinquish Open Bible Baptist Church and its volunteers from any and all liability for injury or accident during the camp or travel to and from the camp.

Deposit

The total cost for the camp is $335. We are asking parents/guardians to pay $55 toward the total camp cost. Open Bible Baptist Church and the youth ministry will fundraise to cover the remaining cost. Cost includes transportation, lodging, meals, and all camp activities.

Deposit is due at registration (right now) and will be credited toward the entire camp cost.

Final payments and outstanding balances are due June 14, 2026.

Fundraising will begin in February 2026. Please be on the lookout for those messages.
